- What is a full PSG?
The gold standard
of sleep investigation.
Polysomnography (PSG) is the most comprehensive sleep study available. It records a wide range of physiological signals simultaneously throughout the night — providing a complete picture of your sleep physiology that simpler studies cannot match.
PSG measures brain activity, eye movements, muscle tone, breathing patterns, oxygen levels, heart rhythm, limb movements, body position, snoring, and continuous video — all in a single overnight study.
Because it captures detailed neurological as well as respiratory data, PSG can investigate conditions that home screening studies cannot reliably diagnose — including narcolepsy, REM sleep behaviour disorder, nocturnal epilepsy, parasomnia, and complex presentations that remain unexplained after simpler tests.
At Grace Sleep, we provide full PSG in the comfort of your own home — avoiding the disruption, unfamiliar environment, and first-night effect associated with hospital-based studies. A trained technician sets up the equipment at your home, monitors the study remotely, and collects the equipment the following morning.
- Why PSG is different
What simpler
studies cannot capture.
Most home sleep studies measure breathing and oxygen — they are excellent for diagnosing sleep apnoea. PSG goes further, adding full neurological monitoring that reveals the structure and quality of sleep itself.
Sleep staging — PSG divides your night into N1, N2, N3 (deep sleep) and REM sleep. This reveals whether you are reaching sufficient deep or REM sleep, and when disruptions occur across the night.
Micro-arousals — brief awakenings too short for the person to notice but sufficient to fragment sleep and cause daytime fatigue. Only detectable with EEG.
Neurological events — abnormal brain activity, REM behaviour disorder, nocturnal epilepsy, and other events that are invisible to respiratory monitoring alone.
Complete muscle data — full EMG set covering limb movements, jaw activity, and chin muscle tone — essential for assessing REM atonia and bruxism.

Neurological
Multiple EEG channels recording brain electrical activity throughout the night. Enables accurate sleep staging, identification of micro-arousals, and detection of nocturnal epileptiform activity.
Respiratory
Nasal airflow thermistor and pressure transducer combined with thoracic and abdominal effort belts — distinguishing obstructive from central apnoeas and detecting hypopnoeas.
Cardiac
Single-lead electrocardiogram recording heart rhythm throughout the night. Identifies cardiac arrhythmias associated with sleep-disordered breathing and other conditions.
Muscle
Submental electromyography recording chin muscle tone. Essential for identifying REM atonia loss — the key finding in REM sleep behaviour disorder — and assessing bruxism.
Video
Overnight video recording synchronised with all physiological channels. Invaluable for parasomnia assessment, nocturnal epilepsy diagnosis, and reviewing any unusual overnight events.
Neurological
Electrooculogram recording eye movements. Essential for identifying REM sleep and distinguishing sleep stages accurately alongside EEG data.
Respiratory
Continuous overnight pulse oximetry recording oxygen saturation and pulse rate. Detects desaturation events and their relationship to breathing and arousal.
Muscle
Tibialis anterior electromyography recording leg movements. Essential for diagnosing periodic limb movement disorder (PLMD) and assessing restless legs syndrome.
Other
Snoring microphone and positional sensor recording snoring intensity and body position throughout the night. Identifies positional sleep apnoea and snoring patterns.
- When is a full PSG recommended?
Conditions that require the full picture.
Full PSG is recommended when symptoms are complex, previous tests have been inconclusive, or there is clinical suspicion of a condition that requires detailed neurological sleep data. Our sleep physicians will advise whether PSG is the appropriate investigation at your consultation.
Narcolepsy
Usually preceded by PSG + MSLT
A neurological sleep disorder causing excessive daytime sleepiness, sudden sleep attacks, disrupted night-time sleep, and sometimes cataplexy. PSG — often followed by a Multiple Sleep Latency Test (MSLT) the following day — is the standard diagnostic pathway.
REM Sleep Behaviour Disorder
Requires video + chin EMG
A parasomnia in which the normal muscle paralysis of REM sleep is absent, causing patients to physically act out their dreams. Diagnosis requires PSG with video and chin EMG to confirm REM atonia loss.
Nocturnal Epilepsy
Requires full EEG + video
Complex Parasomnia
Requires video + full EMG
Sleepwalking, confusional arousals, sleep terrors, sleep talking, and other complex nocturnal behaviours. Video monitoring is essential for characterising the behaviour, while EEG and EMG data allows accurate classification of the parasomnia type.
Idiopathic Hypersomnia
After simpler tests are negative
Excessive daytime sleepiness despite adequate sleep duration, not explained by sleep apnoea or other identifiable cause. Requires PSG to exclude other conditions before diagnosis can be established.
Unexplained Excessive Daytime Sleepiness
When simpler tests are inconclusive
When a patient has persistent, profound daytime sleepiness despite normal sleep duration, negative apnoea testing, and no obvious cause, PSG can identify rarer conditions driving the symptom.
Bruxism
When clinical assessment warrants PSG
Severe or clinically complex teeth grinding and jaw clenching during sleep. PSG with jaw EMG provides a detailed overnight assessment of bruxism frequency, intensity, and relationship to sleep stage and arousal.
Inconclusive Previous Testing
Step-up from simpler studies
When a home sleep study has returned inconclusive results, or symptoms remain unexplained despite appropriate initial testing, PSG provides the additional physiological detail needed to reach a diagnosis.

A prior consultation with one of our sleep physicians is required before a full PSG is booked. This ensures PSG is the most appropriate investigation for your symptoms — and rules out simpler tests that may answer the clinical question at lower cost and complexity. Book a consultation →

On the night of your study, a trained technician visits your home to fit the PSG equipment — including EEG electrodes, EOG leads, ECG, airflow sensors, respiratory belts, EMG leads, and video camera. This takes approximately 90 minutes. Once fitted, you sleep in your own bed as normal.

The PSG equipment records all channels simultaneously throughout the night. The technician monitors the study remotely and returns the following morning to remove the equipment and collect the data.

Your overnight data is scored and reviewed by our clinical team — including manual review of EEG, EMG, respiratory, and video channels. A written report is prepared covering sleep architecture, events, and clinical interpretation.

Your report is discussed in a follow-up consultation with your sleep physician — covering diagnosis, findings, treatment options, and next steps. The follow-up consultation is included in the PSG price.

What is a full PSG?
A full polysomnography (PSG) is the gold-standard sleep study — the most comprehensive investigation available. It records brain activity (EEG), eye movements (EOG), muscle activity (EMG), breathing patterns, oxygen levels, heart rhythm (ECG), limb movements, body position, snoring, and continuous video throughout the night.
Can a full PSG be done at home?
Why does a full PSG require a prior consultation?
A consultation is required to ensure a full PSG is the right investigation for your symptoms. PSG is the gold standard — but for many presentations, a simpler and less expensive study will answer the clinical question equally well. Our sleep physicians will advise the most appropriate test at consultation, which may or may not be a full PSG.
How much does a private full PSG cost?
A full in-home video PSG at Grace Sleep is £2,700. This includes all equipment, the overnight study with technician setup, specialist scoring and reporting, and a follow-up consultation to discuss your results.
How long does the technician take to set up the equipment?
Equipment setup at your home takes approximately 90 minutes. The technician will arrive in the early evening, fit all electrodes and sensors, run a signal check to ensure data quality, and leave once everything is confirmed working correctly. You then sleep in your own bed as normal.
Is PSG appropriate for diagnosing sleep apnoea?
PSG can diagnose sleep apnoea — but for most patients with straightforward symptoms, a simpler home sleep study (Pebble Study at £250 or SMED at £450) is equally accurate, more comfortable, and far less expensive. PSG is recommended for sleep apnoea when simpler studies have been inconclusive, or when complex co-existing conditions need simultaneous investigation.
How quickly will I receive my results?
Full PSG data requires more detailed manual scoring than simpler home studies. Your written report and follow-up consultation will be arranged within approximately 5–7 working days of your study.
Do you refer to NHS neurologists for narcolepsy investigation?
Yes. For narcolepsy investigation, the standard pathway requires PSG followed by a Multiple Sleep Latency Test (MSLT) the following day. Our team will advise on the most appropriate pathway at consultation and can facilitate onward referral to NHS neurological services where appropriate.
